This is accomplished across four geographic management zones which are responsible for the operation of acute care health centres and the provision of a variety of inpatient, outpatient services including academic, tertiary, quaternary care and community based programs and services including continuing care, primary health care, public health, and mental health and addictions. Responsibilities Under the general supervision of the Manager, Continuing Care, the Care Coordinator is responsible for the intake, assessment, service planning and implementation, on going case management, evaluation and appropriate discharge planning of clients within the Continuing Care Program. The Care Coordinator is accountable to administer these functions within the Continuing Care policy, standards and guidelines.
QualificationsBachelor’s Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy or Nutritional studies plus five (5) years directly related experienceor a Master’s Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy or Nutrition plus three (3) years directly related experienceor a Diploma in Nursing plus a Diploma in Public Health Nursing plus five (5) years directly related experienceor Diploma in Nursing plus Diploma in Home Care Nursing (Continuing Care Certificate) plus five (5) years directly related experienceor Diploma in Nursing plus nine (9) years directly related experienceUp to date applicable licensure and NS RegistrationMust have experience across the continuum of care and knowledge of the Continuing Care Program and other community resources.Excellent assessment, interviewing and negotiation skills essential. Strong communication and interpersonal skills. Effective time management and problem solving skills.Must have the proven ability to diffuse difficult and complex situations and be able to respond appropriately.Demonstrated ability to organize and lead case conferences.Effective public speaking/presentation skills.Must have the ability to assess and recognize deficits in normal functioning and be able to encourage self care and independence amongst client groups.Familiarity with computers is essential and knowledge of WordPerfect, E-Mail and keyboarding is an asset.A valid drivers license is required and access to a reliable vehicle may be required.
Hours of Work Short Assignment Part-time (50%) Position; 35 hours bi-weekly Beginning December 6, 2020 with an expected end date of May 31, 2021 These dates are subject to change Salary Information $38.4501 - $43.7766 hourly
